A three-dimensional laser optoacoustic imaging system was developed, which combines the advantages of optical spectroscopy and high resolution ultrasonic detection, to produce high contrast maps of optical absorbance in tissues. This system was tested in a nude mouse model of breast cancer and produced tissue images of tumors and vasculature. The imaging can utilize either optical properties of hemoglobin and oxyhemoglobin, which are the main endogenous tissue chromophores in the red and near-infrared spectral ranges, or exogenous contrast agent based on gold nanorods. Visualization of tissue molecules targeted by the contrast agent provides molecular information. Visulization of blood at multiple colors of light provides functional information on blood concentration and oxygen saturation. Optoacoustic imaging, using two or more laser illumination wavelengths, permits an assessment of the angiogenesis-related microvasculature, and thereby, an evaluation of the tumor stage and its metastatic potential. The optoacoustic imaging system was also used to generate molecular images of the malignancy-related receptors induced by the xenografts of BT474 mammary adenocarcinoma cells in nude mice. The development of the latter images was facilitated by the use of an optoacoustic contrast agent that utilizes gold nanorods conjugated to monoclonal antibody raised against HER2/neu antigens. These nanorods possess a very strong optical absorption peak that can be tuned in the near-infrared by changing their aspect ratio. The effective conversion of the optical energy into heat by the gold nanorods, followed by the thermal expansion of the surrounding water, makes these nanoparticles an effective optoacoustic contrast agent. Optical scattering methods and x-ray tomography may also benefit from the application of this contrast agent. Administration of the gold nanorod bioconjugates to mice resulted in an enhanced contrast of breast tumors relative the background of normal tissues in the nude mouse model. The combination of this novel contrast agent and optoacoustic imaging has the potential to become a useful imaging modality, for preclinical research in murine models of cancer and other human diseases.

Thermal therapy using various heating sources such as lasers or microwaves to destroy benign and malignant lesions has recently gained widespread acceptance. However, the accurate prediction of thermal damage in tissue according to theoretical or computer modeling is difficult and unreliable due to target variability with respect to physical properties, geometry, and blood perfusion. Thus, one of the major obstacles to application of thermal therapies has been the lack of a noninvasive, real-time method that could determine the extent and geometry of treated tissue. To evaluate the effects of laser heating on tissue, we have developed an analog-digital hybrid Doppler ultrasound system to measure the phase and amplitude of ultrasonic echoes returned from the heated tissue. The system consists of an eight-gate pulsed Doppler detector, a 16-channel 12-bit A/D converter, and a signal analysis and visualization software package. In vitro studies using canine liver showed two distinct types of modulation of the echoes along the ultrasound beam path during laser irradiation using an 810 nm diode laser. Type 1 signals showed a small and slow variation in amplitude and phase, and were attributed to tissue coagulation. Type 1 signals showed a small and slow variation in amplitude and phase, and were attributed to tissue coagulation. Type 2 signals showed large and rapid variations in amplitude and phase which usually appeared after tissue surface explosion and were indicative of tissue ablation. We hypothesize that the observed phase changes in type 1 signals are due to thermal effects within the tissue consistent with tissue expansion and contraction while the phase changes in type 2 signals are likely due to formation and motion of gas bubbles in the tissue. A further development of the Doppler ultrasound technique could lead to the generation of feedback information needed for monitoring and automatic control of thermal treatment using various heating modalities such as laser, high intensity focused ultrasound, microwaves, or radio frequency waves.

Insufficient light absorption in hard dental tissues makes laser ablation in near UV, visible or near IR spectral ranges very inefficient to be employed for tooth cavity preparations. We used deposition of a liquid absorber, indocyanine green (ICG) dye, to overcome this problem. Experiments employed Alexandrite laser anticipating that future near IR diode laser technology will replace existing medical lasers. Ablation kinetics and mechanisms for both free-running and Q-switched modes of Alexandrite laser were studied with the aim to determine optimal parameters of laser irradiation and optimal volume of the dye. Four experimental parameters were monitored during each ablation event: (1) incident laser fluence, (2) temporal profile of the laser pulse, (3) temporal profile and magnitude of laser-induced stress transients, (4) temporal profile and spectrum of plasma emission. We also examined kinetics of plume by probing ablation products with CW He-Ne laser beam. Results depicted ablation process as a complex multistage phenomenon. Two distinct stages associated with the tooth ablation are revealed in the free-running mode: (1) ablation of a dye droplet from a tooth surface by the first laser micropulse of a 250-microsecond(s) macropulse, (2) plasma mediated ablation of a melted layer of enamel produced by thermal explosion of the dye. Plasma jet formation was delayed 10-100 microsecond(s) against the beginning of free-running pulse. Ablation stages and their efficiency are defined by laser irradiation parameters, dye concentration and its total volume. In contrast, Q-switched (nanosecond) laser ablation occurs as a one stage process, and, therefore, less efficient. In addition, Q-switched mode irradiation induces shock waves amplitudes that are about an order of magnitude higher compared with that induced by the free-running irradiation. Experimental comparison of Q-switched and free-running modes of irradiation is evident in favor of free-running mode that produces a nice smooth crater without noticeable thermomechanical damage to surrounding tissues.

Mechanical injury during pulsed holmium laser ablation of tissue is caused by rapid bubble expansion and collapse or by laser-induced pressure waves. In this study the effect of pulse duration on the photomechanical response of soft tissue during holmium:YAG laser ablation has been investigated. The dynamics of laser-induced bubble formation was documented in water and in transparent polyacrylamide tissue phantoms with a water concentration of 84%. Holmium:YAG laser radiation ((lambda) equals 2.12 micrometers ) was delivered in water or tissue phantoms via an optical fiber (200 or 400 micrometers ). The laser was operated in either the Q- switched mode ((tau) _p equals 500 ns, Q_p equals 14 +/- 1 mJ, 200 micrometers fiber, H_o equals 446 mJ/mm²) or the free-running mode ((tau) _p equals 100 - 1100 microsecond(s) , Q_p equals 200 +/- 5 mJ, 400 micrometers fiber, H_o equals 1592 mJ/mm²). Bubble formation was documented using a fast flash photography setup while simultaneously a PVDP needle hydrophone (40 ns risetime), recorded pressures. The effect of the pulse duration on the photomechanical response of soft biological tissue was evaluated by delivering 5 pulses of 800 mJ to the intimal side of porcine aorta in vitro, followed by histologic evaluation. It was observed that, as the pulse duration was increased the bubble shape changed from almost spherical for Q-switched pulses to a more elongated, cylindrical shape for the longer pulse durations. The bubble expansion velocity was larger for shorter pulse durations. A thermo- elastic expansion wave was measured only during Q-switched pulse delivery. All pulses that induced bubble formation generated pressure waves upon collapse of the bubble in water as well as in the gel. The amplitude of the pressure wave depended strongly on the size and geometry of the laser-induced bubble. The important findings of this study were (1) the magnitude of collapse pressure wave decreased as laser pulse duration increased, and (2) mechanical tissue damage is reduced significantly by using longer pulse durations (> 460 microsecond(s) , for the pulse energy used).
